Xponential CAP Connection Sign Up

In order to set up your account, please follow these steps:

1. You will need to get setup with a secure CAP connection by filling out the CAP Connection Form. You will need to fill out a separate form for each location you wish to connect.
2. There are 5 steps on the CAP Connection Form. All of them must be completed in order to connect your ad account.
3. Complete the form below, and attach the completed CAP Connection Form. You only need to fill out the form once, but please list all locations you are connecting in the field, and include all Connection Forms. Please email us before filling out this form if you have any questions.
